We encourage you to come in for a tour so you can meet our staff and see first-hand what makes Sprouts About such a great option for busy families. A tour not only answers many of your questions, but also helps ease the transition for your children during their first visit. A staff member will go over the various registration forms and answer any further questions you may have. Because we are a DCFS licensed child care center, we require various forms to be filled out prior to allowing any children to stay at our center. The registration forms are listed below.
